Promoting Independent Living
Solomon Home Health Care is a full-service home health care provider. We help elderly and disabled individuals in Milwaukee Metropolitan Area who require assistance to remain in their own homes and are not yet ready to move to an assisted living facility or nursing home. Through hands-on guidance and monitoring, we make it possible for our clients to enjoy independent living all the while having peace of mind knowing quality home health care is within reach at all times.
Skilled Nursing
Solomon Home Health Care has a Home Health Care component “Solomon Home Health Care’ in which we provide a wide range of health care services that can be given in your home for an illness or injury which requires skilled nursing. The client must have Medicare Insurance to qualify for this in-home service. Part-time or intermittent nursing care is skilled nursing care you need or get less than 7 days each week or less than 8 hours each day over a period of 21 days (or less) with some exceptions in special circumstances.
We are in the process of getting Medicare Accreditation from ACHC to provide this service.
Skilled Nursing includes:
- Wound care for pressure sores or a surgical wound
- Patient and caregiver education
- Intravenous or nutrition therapy
- Injections Skilled Nursing helps you:
- Get better
Regain your independence
- Become as self-sufficient as possible
- Maintain your current condition or level of function
- Slow decline
Supportive Care
Supportive Home Care is provided to elderly and disabled persons residing in their own homes; consists of assistance with daily living needs, including household care, physician appointment, grocery shopping. The client must be participating in the IRIS Program to be eligible for these services. Please call or detail on how to apply.
Personal Care
Personal Care is for someone who is disabled or is otherwise unable to care for themselves that consist of ADLs within the residence. The client must have Medicaid Insurance to qualify for this in-home service. Under Personal Care, potential clients could have a family member or friend be trained to be their caretaker.
Activities of daily living (ADLs) are routine tasks that people must carry out each day to take care of themselves. The ADLs used by most systems fall into six main types:
- Eating: assistance meal preparation and supervision
- Bathing: assistance tub and shower
- Grooming: assistance oral care, hair care, shaving
- Dressing: assistance Choose to clothe and get dressed Buttons
- Mobility (or “transfer”): assistance with getting into and out of bed or a chair, and assistance with moving from place to place with the home.
- Continence: assistance with bladder and bowel functions
- Toileting: assistance with getting on and off the toilet
Non-Medical Transportation
SMS – Transportation. Providing caring and responsible wheelchair and ambulatory transportation for patients to and from their medical appointments, and other events using a company-owned non-emergency medical transportation vehicle in Milwaukee County, Milwaukee, Wisconsin. Coming Soon!
